Forms

What follows is a list of the forms you'll need to fill out and submit as you participate in your internship. It's important that you submit them when they are due because they tell me how your internship is going and alerts the internship coordnator to any problems that may require his help.

All forms are in .pdf format.

Please submit the reports according to these deadlines:

    • The Request to Intern form is due before the semester begins.
    • The Employer-Student Agreement is due sometime within the first two weeks of class (in the first week of a summer session).
    • The Progress Report (one from the student and another from the intern sponsor/ employer) is due halfway through the semester or summer session.
    • The Final Report (both the student's and the employer/sponsor's) is due on the last day of classes of any semester or summer session.

Note: Interns are responsible for reminding the intern employers to submit their forms on time. All forms must be submitted before any intern will be granted internship credit at the end of the semester.
