Use this form to add or drop a major or minor within CLAHS. Add or drop majors or minors within the appropriate departments.
Authorization to Take Courses Elsewhere Form (within the US)
Complete & submit this form BEFORE you take courses elsewhere! Download, complete the top portion, fill out the left half of the table at the bottom (info on courses you are taking elsewhere). Bring it to Cathy Skinner in Shanks 329 for review & signature, then take it to the CLAHS Undergraduate Affairs Office for processing (Wallace 238).

Course Withdrawal Form
Use a Course Withdrawal form to withdraw from a course after the regular drop deadline. According to college policy, such forms are no longer available online and must be picked up in Shanks 329.
If you want to take more than 19 hours of coursework in any given semester, you will have to make a formal request for overhours or an overload. Complete the form, bring it to Cathy Skinner in Shanks 329 for a signature, and then take it to the CLAHS Undergraduate Affairs Office for processing (Wallace 238).
Use this form if you and a teacher plan to set up an independent study (design your own course and work individually with a teacher). Complete the top portion with the teacher, include a separate course syllabus (see form for details), then bring it to Cathy Skinner in Shanks 329 for a signature. It will then be sent to Nancy Metz, Associate Chair of English. If it is approved, it will be sent to the CLAHS Undergraduate Affairs Office for processing (Wallace 238). The CLAHS UAO is responsible for adding it to your schedule.

Student Resignation/Withdrawal Form
Use this form if you need to withdraw from all of your courses any given semester due to illness or extenuating circumstances OR if you need to drop your last class (Hokie SPA will not let you drop all coursework any given semester). Download the form, complete it, get all relevant signatures (directions are on the form), then take it to the University Registrar in Student Services Building 250.
